Heartland Hospice

Heartland Hospice provides care for clients during the last stage of an  incurable illness. The program is designed to give physical, emotional, and spiritual support to clients, as well as families and caregivers who choose to provide care for their loved ones.

 

Services Available:

Nurse – Assesses for and strives to alleviate physical signs and symptoms of disease process.  They offer a vital role within the Hospice Team to coordinate care.

Home Health Aides –  Assist with everyday personal care such as bathing, skin care, and light housekeeping tasks that help relieve some stress from family members.

Social Workers –  Provide emotional support to client and family through counseling and relaxation techniques.  Assist with guidance for any legal questions, funeral planning and community resources.

Spiritual Care – To assist clients in exploring their spiritual journey, identifying their own purpose and worth, offer support during disease process to clients and family.

Volunteers –  Strive to offer more one-on-one companionship and can provide some respite for family.  From assistance with running errands to holding  the hand of a client who is scared.

Referral Process

Anyone may refer a patient to Hospice. Referrals usually come from a physician, family member, clergy, or health care professional. All referrals will be discussed with the client's attending physician before other action is taken. 

Payment For Services

Fees may be collected from Medicare, Medicaid, and Veterans Administration (VA) if criteria is met.  Insurance policies may be  researched for hospice coverage provisions.
