Thank you for your interest in being a vendor at the MCMH Live Well Farmers Market!

  Our goal is to create a market that serves as a vibrant hub for local producers and food enthusiasts alike, fostering a community-driven approach to healthy living and sustainable agriculture and its connection to our health! 

  • We require all vendors to be in compliance with the State of Iowa farmers market licensing requirements.
  • We are a producer market, meaning, that vendors must either grow or make what they sell. Please note that we exclusively accept vendors who primarily offer food-based products.
  • To participate in our market, vendors must submit a farmers market application for approval prior to selling, sign a vendor agreement and provide any necessary licenses.

Terms and Conditions:

  • Markets will take place inside the MCMH + Clinics conference center (which means temperature-controlled environment and markets happen rain or shine!)
  • Vendors will have the option to choose a 1 table or 2 table space. Tables are 35.5" x 71"
  • All vendors must be approved prior to selling at the MCMH Live Well Farmers Market
  • There is NO vendor fee to participate in the MCMH Live Well Farmers Market 

Responsibility Waiver:

For any illness, injury, property damage, or loss suffered or sustained by me which is in any way associated with my participation in, travel to and from, or other activities associated with the MCMH Live Well Farmers Market , I do hereby, for myself, my heirs, and my administrators and executors, forever waive, release and discharge any and all rights and claims for expenses, damages, or other losses which I may have or which may hereafter accrue, against Montgomery County Memorial Hospital, their sponsors, officers, directors, employees, agents, successors, and assigns.

Contamination Affidavit

The undersigned vendor states that all products offered for sale by this vendor at the MCMH Live Well Farmers Market are free of contamination from chemical residues and bacterial sources. The undersigned vendor further states that all pesticides and herbicides used in the production of items offered for sale by this vendor have been applied in accordance with current rules of the US Environmental Protection Agency, US Food and Drug Administration, and the Iowa Department of Agriculture.

Hold Harmless Agreement

Vendor agrees to indemnify and hold harmless Montgomery County Memorial Hospital and its employees, volunteers and sponsors from any and all liabilities and causes of action which may arise by virtue of vendor’s participation in the MCMH Live Well Farmers Market.

 


After your application is reviewed you will be contacted by either Live Well Montgomery County or Montgomery County Memorial Hospital. 

 


The following products must have a license to be sold at farmer's markets:  Unpackaged foods that are temperature-controlled for safety, which include: Meat Poultry Dairy products Cooked or prepared foods Soft pies Custard-filled products Cream-filled products; Wild-harvested mushrooms as described in 481 Iowa Administrative Code, Section 31.1(4), Wild-harvested mushrooms.

If you answered yes to the question above, please refer to Iowa inspections and Appeals Farmers Market Requirements and Iowa Code Chapter 31. You will be required to provide a copy of your license.
